Outpatient clinics provide traditional outpatient services such as individual, group, family, rehabilitation, case management, and psychiatric / medication support services at locations throughout the county, often on a referral basis.
Partners Active Care Team (PACT) is a collaborative of Child and Family Teams, Intensive Care Coordination, and Intensive Home Based Services. Eligible youth are 21 and under and have intense behavioral health needs.
The goal of these programs are to enhance child and youth safety, permanency, and well-being through long-term stability in treatment and natural supports and community resources.
