Welcome to Older Adult Services
San Joaquin County Older Adult Services recognizes that
the quality of life for many 60 year olds and older can be
enhanced through an array of services. Our programs are
designed to provide support and assistance in maintaining
independence. For those who can no longer live
independently, even with in-home assistance, we offer
programs to help ease the transition to long term care.

case management, assessment, individual therapy,
resource and referral, individual therapy services,
consultation with family members and support system of
consumer. |

Provides
monitoring and follow-up for consumers 60 years and
older, who are unable to manage their financial
affairs, have a history of mental illness or under
conservatorship, includes sub-payeeship
services. Services are provided to consumers
residing independently, living in board and care, and
skilled nursing facilities. |

| Description: |
Staff psychiatrists perform psychiatric evaluations,
provide medication monitoring, medication education,
as well as evaluate for physical medical difficulties
and interaction with psychiatric medications and
physical medications. Staff psychiatrists communicate
with primary care physicians and other medical
professional in the community as well as family
members.
