San Joaquin County Older Adult Services recognizes that the quality of life for many 60 year olds and older can be enhanced through an array of services. Our programs are designed to provide support and assistance in maintaining independence. For those who can no longer live independently, even with in-home assistance, we offer programs to help ease the transition to long term care.

Case Management for Older Adult Services

Program: Case Management for Older Adult Services
1201 N. El Dorado Street
Stockton, CA 95202
Type of Program: Outreach, case management, assessment, individual therapy, resource and referral, individual therapy services, consultation with family members and support system of consumer.
Description: Provides monitoring and follow-up for consumers 60 years and older, who are unable to manage their financial affairs, have a history of mental illness or under conservatorship, includes sub-payeeship services.  Services are provided to consumers residing independently, living in board and care, and skilled nursing facilities.

Medication Clinic

Program: Medication Clinic
1201 N. El Dorado Street.
Stockton, CA 95202
Type of Program: Psychiatric evaluation, medication support services, and monitoring of medications
Description: Staff psychiatrists perform psychiatric evaluations, provide medication monitoring, medication education, as well as evaluate for physical medical difficulties and interaction with psychiatric medications and physical medications. Staff psychiatrists communicate with primary care physicians and other medical professional in the community as well as family members.

Transitional Care Facility

Program: Transitional Care Facility
1201 N. El Dorado Street.
Stockton, CA 95202
Type of Program: Provides temporary supportive care to abused, endangered or abandoned older adults in Transitional Care Housing.
Description: Joint program with HSA, Dept. of Aging and Mental Health. Provides temporary safe environment for up to 14 days.

Older Adult Day Treatment

Program: Older Adult Day Treatment

Type of Program: Specialized program to assist older adults 60+ years of age who living in the community by providing support and group and individual counseling. 
Description: Groups include: psychotherapy, music, creative arts, Spanish speaking, African American, Adult Children Caring for Aging Relatives, and Medication Education.

Program: Older Adult Senior Peer Counseling Program
