Application Fee: $50.00 (effective July 1, 2008)
Late Fee: $20.00 for an application submitted after lapse (effective July 1, 2008)
* Note: Live Scans processed more than 90 days in advance of submitting the EMT-I Certification Application will have to be repeated at the applicants cost.

Frequently Asked Questions
What do I need to apply for EMT-I certification in San Joaquin County (SJC)?
Initial Certification: If this is your first time applying for certification you need to submit the following:
- Application with affidavit
- Copy of EMT-I course completion certificate
- Copy of proof of passing pre-established written exam (National Registry card)
- Copy of current Professional Rescuer/Health Care Provider CPR/AED card
- Copy of a valid CA driver’s license, CA identification card, or military ID
- Application fee
- The applicant must have a DOJ live scan finger printing background check done for this agency
Reciprocity Certification: If the applicant is certified in another county and would like to become certified in San Joaquin County,
the following needs to be submitted:
- Application with affidavit
- Copy of valid EMT-I certificate
- Copy of current Professional Rescuer/Health Care Provider CPR/AED card
- Copy of a valid CA driver’s license, CA identification card, or military ID
- Application fee
- EMT-I Certification Verification Form
- The applicant must have a DOJ live scan finger printing background check done for this agency
Re-certification: If the applicant is re-certifying, the following must be submitted:
- Application with affidavit
- Copy of 24 hours of continuing education certificate(s)
- Completed original EMT-I skills verification sign-off form
- Copy of current Professional Rescuer/Health Care Provider CPR/AED card
- Copy of a valid CA driver’s license, CA identification card or military ID
- Application fee
- If the applicant does not already have a DOJ live scan finger printing background check on file for this agency the applicant must
obtain one
Can I become certified if my EMT-I has expired?
- If the applicant’s certification has lapsed less than 6 months just submit all the items required for re-certification along with the late fee.
- If the applicant’s certification has lapsed is beyond 6 months BUT less than 12 months the applicant needs to submit all items required for
re-certification plus 12 additional hours of EMS Continuing Education (for a total of 36 hours of CE), this includes the late fee
- If the applicant’s certification lapsed beyond 12 months BUT less than 24 months the applicant needs to submit all items required
for re-certification plus 24 additional hours of EMS Continuing Education (for a total of 48 hours of CE) and proof of passing
the written and skills examination required for initial certification (National Registry), this includes the late fee
- If the applicant’s certification has lapsed beyond 24 months the applicant must complete all requirements of initial
certification including successful completion of an approved EMT-I training course
Why do I need to print 3 copies of the Live Scan Request Form?
You need 3 copies for distribution as follows:
- Live Scan Operator
- San Joaquin County EMS Agency
- Your copy
The copy you give to the San Joaquin County EMS Agency and the copy you keep for yourself can be used to track what
happen if your Live Scan fails to be processed.
