1.                  bos - Approve the Appointment of 1 Position to the Local Child Care Planning Council: (1) Discretionary Representative.

CONSENT - HEALTH & HUMAN SERVICES

1.                  bhs - Approval of 2013-2014 Agreements with BHS Heritage Oaks Hospital, Inc., John Muir Behavioral Health Center, St. Helena Hospital Center for Behavioral Health, BHC Sierra Vista Hospital, Inc., and Sutter Center for Psychiatry for $815,000 to Provide Inpatient Psychiatric Treatment Services.

2.                  hsa - Approval of Agreement with San Joaquin County Office of Education to Support the Juvenile Dependency Court from January 1, 2014 through June 30, 2015 for $130,030.

3.                  phs - Approval of Revised Public Health Emergency Preparedness Allocation Agreement with the California Department of Public Health for the Period July 1, 2013 through June 30, 2014 with a Maximum Payable of $1,009,050.

4.                  sjgh - Approval of Applications for Appointment to San Joaquin General Hospital Medical Staff.

5.                  sjgh - Approval of Applications for Appointment/Reappointment to San Joaquin General Hospital Allied Health Staff.

6.                  sjgh - Approval of Physician Employment Agreement with Dr. Bi for the Department of Internal Medicine, Gastroenterology Services Effective December 9, 2013 through June 30, 2014 for an Annual Salary of $353,216.

7.                  sjgh - Approval of Second Modification of Professional Services Agreement with APEXX Physicians Medical Corporation, Inc. for a Three Month Extension through March 30, 2014 in an Amount not to Exceed $3,500,000 per Fiscal Year. 

8.                  sjgh - Approval of Amendment No. 12 to Agreement with the Regents of the University of California for Gynecological-Oncology Services for the Period October 1, 2013 through September 30, 2014 in the Amount Not to Exceed $39,600.

9.                  sjgh - Approval of Amendment No. 6 to the Agreement with the Regents of the University of California for Affiliation for Medical Education, Effective September 16, 2013 through June 30, 2014.

CONSENT - PUBLIC WORKS

10.              pw - Approval of Agreement for the Removal or Replacement of Existing Facilities with Lincoln Properties for the Benjamin Holt Drive Improvement Project.  (2nd District)

 

11.              pw - Approval of a Fourth Time Extension to the Agreement with More-Gas Investments, LLC and Moreno Trenching, LTD., Inc., to Complete Subdivision Improvements for Tract No. 3560, "Tom's Place" Subdivision, Located on the Southwest Corner of Jahant and Tretheway Roads Extending Completion Date from December 9, 2013 to December 9, 2015.  (4th District)

12.              pw - Adopt a Resolution Making Certain Findings Regarding Projected Cost Savings for the Hazelton Corporation Yard Solar Carport Project.

CONSENT - LAW & JUSTICE

13.              s/o - Approve and Authorize Sheriff to Execute Lease Agreements with Menlo Pointe Apartments to House Deputy Sheriffs while Attending the Butte College POST Academy.

14.              s/o - Approve Amendment to Contract with J. Morris Company for Body Removal Services to Increase Not to Exceed Limit from $100,000 to $200,000 Annually.

CONSENT - GENERAL GOVERNMENT

15.              cdd - Introduce and Waive Reading of the Ordinances Regarding the 2013 California Building Code, 2013 California Residential Code, 2013 California Electrical Code, 2013 California Mechanical Code, 2013 California Plumbing Code, 2013 California Green Building Code and the 2013 California Fire Code, Hereinafter Referred to as 2013 California Code of Regulations Title 24.  (Said Ordinances to be Adopted on December 10, 2013)

16.              cob - Approve the Announcement of 1 Position to the San Joaquin Regional Transit District Board: (1) Stockton City Council/San Joaquin County Board of Supervisors Joint Appointee.

17.              gen/ser - Approve the Transfer of Park Subdivision Trust Fees to the Linden Unified School District in the Amount of $3,309.

18.              gen/ser - Authorize the Release of Request for Proposals (RFP) for Regional Sports Complex Maintenance, Operations, and Concession Lease.
