CONSENT - HEALTH & HUMAN SERVICES

1.                  hcs - Approval of the Low Income Health Program Data Use Agreement with the California Department of Health Care Services for Data Access and Exchange for January 1, 2014 through December 31, 2014.

2.                  sjgh - Approval of Appointment/Reappointment to San Joaquin General Hospital Medical Staff.

3.                  sjgh - Approval of Appointment to San Joaquin General Hospital Allied Health Professional Staff.

4.                  sjgh - Approval of Expansion of Privileges to Include Emergency Medicine for San Joaquin General Hospital Allied Health Professional Staff.

5.                  sjgh - Approval of Additional Privileges to San Joaquin General Hospital Allied Health Professional Staff.

CONSENT - PUBLIC WORKS

6.                  pw - Approval and Adoption of Plans and Specifications for the Northwest Tracy Streets Resurfacing and Setting a Bid Opening Date of January 30, 2014, at 1:30 P.M.  (5th District)

7.                  pw - Adoption of an Initial Study/Negative Declaration for Balboa Avenue Interconnection Project.  (3rd District)

CONSENT - GENERAL GOVERNMENT

8.                  ag/com -  Approval of the 2013-2014 Agreement with California Department of Food and Agriculture for Dog Team Program for the Period of July 1, 2013, through June 30, 2014, totaling $148,556.

9.                  ag/com - Approval of the 2013-2014 Agreement with California Department of Food and Agriculture for High Risk Pest Exclusion Program for the Period of July 1, 2013, through June 30, 2014, totaling $28,535.

10.              bos - Approval of the Board of Supervisors' Committee Assignments for 2014.

11.              cao - Approval of Revisions to the San Joaquin County Employees' Retirement Association Bylaws.

12.              cob - Approve the Announcement of 1 Position to the Workforce Investment Board:
(1) Third Supervisorial District Representative of Local Business.

13.              h/r - Implement Classification Project #13-024 by Creating, Assigning, and Allocating the Class of Recorder-County Clerk Operations Supervisor to the Recorder-County Clerk Budget and Deleting a Vacant Administrative Assistant I Position.

14.              h/r - Assign the Classes and Multi-Allocate and Block Budget One (1) Position of Administrative Assistant I/Junior Administrative Assistant, Assign the Class and Allocate One (1) Position of Executive Secretary; Delete One (1) Position of Administrative Assistant II, and Delete One (1) Multi-Allocated Position of Senior Office Assistant/Office Assistant in the Sheriff's Department Budget.
